The GBD Project has characterized and captured data on disease-related mortality, morbidity and disability since the early 90s. As an engineer, I appreciate the expansive effort to better understand the burden of disease in a data-driven manner; however, the existence of data, let alone complete data, is a luxury found in high-income and some middle-income countries. Where does that leave the rest of the world? In 2010, only 15% of the world’s population was captured by complete, high-quality data. For some countries, their incomplete datasets were sufficient enough to allow for extrapolation utilizing the country-based data. Most of the world, however, lacks any such viable data and the result is extrapolating on extrapolated data that was extrapolated from other extrapolated data.
Adult mortality in these countries is extrapolated utilizing in-country child mortality and an estimated median level of adult mortality from complete, historical life tables from across the world. These life tables could be from a different time in history and/or a different part of the world than the LMICs whose data are being extrapolated from these life tables. These estimated values are then compared to modified life tables in order to find a “best” match and create a dataset consisting of age-specific death rates. This methodology is used for general mortality distributions. One step further allows for the creation of disease-specific mortality distributions. The vast amount of estimation and correlation utilized in order to calculate these numbers introduces an extremely high amount of uncertainty for the GBD reported adult mortalities and causes of death, especially for non-communicable diseases which are less understood than communicable diseases.
Despite, the large amounts of estimation in the data, the GBD project does not caution about the uncertainty associated with this data. In fact, one has to dig in order to find the data extrapolation methods, and I still have not been able to find clear uncertainty values associated with the distributions that they report. Data without clearly stated limitations or uncertainty is troublesome. For some individuals, the extent of extrapolation would warrant calling the data fabricated, especially when reported without caution or uncertainty values. To be clear, the GBD project is an excellent project that will, eventually, serve as a great tool for policymakers across the world. But rather than spending vast amounts of money on extrapolation tools for LMICs, should we be redirecting those specific funds towards helping these countries establish more reliable means of collecting death-related data? Even if the data is incomplete, utilization of country-based adult mortality data has to be better than the degree of extrapolation that currently exists in order to estimate the impact of NCDs on mortality, morbidity and disability.

Currently, has there been an improvement in the transition from acute to chronic care models in the countries that you have researched in? And if so, what specifically has changed?
Apart from systematically improving training and allocation of resources at the patient health care and health care organizational level for chronic care, do you believe there is a space for mobile health to aid this improvement in the near future from your past experiences? If so, what experiences have you had or papers have you read that portray the challenges and/or success to addressing chronic conditions with eHealth in LMICs?
